You need to fill form 6 and submit it with address proof  to add your name to the new electoral list. Please download Form 6 attached, fill it and submit it with one of the address proof in your Assembly Constistuency 

Address Proof Documents:
1. Passport
2. Telephone Bill [ Mobile Bill also]
3. Water Bill
4. Electricity Bill
5. Rental Agreement
6. Any identification card issued by the Govt/ Post office
7. Valid Driving License
NOTE: address mentioned in the address proof and the application has to match.
